City of Corvallis Social Service Funding

On an annual basis, the City of Corvallis allocates Funding to social service organizations serving the needs of low income families/individuals living in the City of Corvallis. This Funding is available to organizations who service clients within or from the CITY OF CORVALLIS CITY LIMITS ONLY. Annually funding is distributed in accordance with cc 00.6.05 Social Service Policy. 

Eligibility 

​Organizations applying for City social service funding must be recognized as a non-profit by the Federal Government with a 501(c)(3) tax-exempt status certification or be a governmental or quasi-governmental agency.

Social Service Impact Areas 

Food Security
Programs dealing primarily with hunger relief, nutrition, and food insecurity issues for any population.

Human Services
Programs addressing physical, mental, social, and/or emotional health, and programs offering services for victims of violence or abuse. 

Intervention and Prevention services
Programs Helping families meet their needs and offer a wide variety of support for families. As prevention encompasses a broad array of services and interventions for families, these services are short-term, family-focused services designed to assist families in crisis by improving parenting and family functioning while keeping families safe.

Shelter Services
Programs providing shelter, temporary housing, and/or other services for temporarily homeless persons.

Shelter Support Grantees ​

It is a requirement of ​grantees that shelter unhoused individuals participate in Coordinated Entry and provide daily shelter use. Grantees must agree too:  

HOMELESS MANAGEMENT INFORMATION SYSTEM AND COORDINATED ENTRY. 

​A. Data Entry into Homeless Management Information System (HMIS). Recipient shall enterinformation into Homeless Management Information System (HMIS) (currently ServicePoint) on all people served with this funding. Data requirements include:
  1. Client data must be complete, accurate, and entered into the database within 48 hours of client’s enrollment or admittance to shelter; and
  2. Recipient must have a secure IT network, be able to pay for the required ServicePoint  license fees, and employ staff to do data entry after receiving proper training through the System Administrator for the Rural Oregon Continuum of Care (ROCC): ROCC HMIS Lead: David Mulig, [email protected]
B. Coordinated Entry. By January 1, 2023, implementation will begin as follows: Recipient must determine if the client has been given a Coordinated Entry (CE) Assessment by reviewing the client records in HMIS (currently ServicePoint). If no assessment has been done, Recipient staff shall attempt to complete the CE Assessment with unhoused clients who have utilized services for 7 or more days in a month. If the guest agrees, the full assessment, including the Base Assessment and Vulnerability Assessment (currently the VI-SPDAT v. 2) must be entered into ServicePoint as an enrollment under the Community Services Consortium (CSC) Coordinated Entry Assessment provider or other designated entity. If the client refuses, the refusal should be noted. If the Recipient staff is not trained to administer the Coordinated Entry Assessment, they should refer the guest to a trained provider such as Community Services Consortium to have the assessment completed with the client.
  1.  Coordinated Entry Utilization. Should Recipient provide microshelters, transitional housing, and/or permanent supportive housing, they must utilize the local Coordinated Entry referral list to find applicants for all of their project vacancies. Recipient shall offer placements to those who score in the appropriate range for the project as determined by the sanctioned vulnerability assessment tool in use (currently the VI-SPDAT v. 2). For example, permanent supportive housing slots should be offered to those who score in the Permanent Supportive Housing (PSH) range of the VI-SPDAT. Selected applicants must also meet the written eligibility criteria established by the Recipient for project participants. Recipient shall not be required to offer application to those on the Coordinated Entry referral list who do not meet the requirements for their project (i.e., a project designated for families with children shall not be required to offer placement to single adults regardless of vulnerability score). Recipient must be able to keep client records (both electronic and hard copy) in a safe, secure manner.
  2. Daily Summary of Number of Individuals Utilizing Beds and Unused Beds. In addition to the above reporting requirements, Recipient shall provide a daily summary of the number of DocuSign Envelope ID: 9B7C8FBE-86D9-46E4-B9CD-35376A2EDFEA FY 22-23 CDBG Human Services Fund Funding Agreement: JSYS Page 12 of 15 individuals who utilized shelter beds, transitional housing and/or microshelters the previous night and the number of unused beds the previous night. Reporting will be due Monday-Friday at a time to be determined.
