Linn, Benton, and Lincoln Counties 
2025 Modernization Grant

Purpose and Goals

The purpose of this grant is to facilitate the equitable distribution of funds among community-based organizations and agencies that support the goals and strategies outlined in the R-CHIP. The key objectives include:

  • Strengthening the R-CHIP goals and strategies within Linn, Benton, and Lincoln Counties.
  • Ensuring a cohesive and complementary approach to addressing public health priorities.
  • Supporting data-driven, evidence-based strategies.
  • Expanding the awareness and implementation of the R-CHIP across agencies and organizations in Linn, Benton, and Lincolin Counties.

Eligibility 

Applicants are encouraged to align their proposals with key strategies from the RCHIP that are best suited for this funding opportunity, including but not limited to: The following entities are eligible to apply:
  • 501(c)(3) Nonprofit organizations
  • Faith-based organizations, including churches
  • Government entities, including municipalities and public health departments
  • Organizations working in the Linn, Benton, and/or Lincoln County Regions 

About Partnership for Community Health

About the Partnership for Community Health:

The Partnership for Community Health of Linn, Benton & Lincoln Counties (PCH) is a multiagency collaborative of local public health authorities, health systems, and key partners committed to improving community health in the region. The PCH works together on the regional Community Health Assessment and Improvement Plans to share information, enhance collaboration with community agencies, and elevate common health-related needs in the region.

What is Public Health Modernization?

Modernization aims to ensure that the public health system operates efficiently, aligns with health system transformation, and provides critical health protections for every person in the state. The Oregon State Legislature funds local public health authorities and regional partnerships to increase their capacity to achieve a high standard of health for all Oregonians.

Priority Areas

Proposals must align with at least one of the following R-CHIP priority areas:
  • Access to Affordable Housing
  • Access to Quality Care
  • Behavioral Health
  • Inclusion, Diversity, Anti-Racism, and Equity (IDARE)

Funding Areas 

Grant Amount: Up to $200,000+ for Linn or Regional Work (Must be inclusive of Linn County)

$25,000 Programmatic Grants (4 Awards):

Supports specific projects, services, or initiatives that directly address community health priorities and provide measurable outcomes. These funds are intended for direct service delivery, community education programs, and outreach efforts.​
​$50,000 Capacity-Building Grants (2 Awards):
​

Designed to strengthen organizations by enhancing their infrastructure, workforce, partnerships, or long-term sustainability. These funds can support activities such as staff training, technology upgrades, and strategic planning. This funding supports specific projects, services, or initiatives that directly address the community health priorities and provide measurable outcomes.
​Grant Amount: Up to $60,000 for Lincoln, Benton or Regional Work (Must be inclusive of Benton and Lincoln Counties)
$15,000 Programmatic Grants (4 Awards):
​

Supports specific projects, services, or initiatives that directly address community health priorities and provide measurable outcomes. These funds are intended for direct service delivery, community education programs, and outreach efforts.
Clarifying Regional Eligibility:
Funding is available for both county-specific and regional efforts—with the important requirement that funded work must include meaningful activities in one or more of the priority counties, as outlined below: 
  • Linn County or Regional: Organizations may apply for funding to support work that takes place specifically in Linn County or spans the broader region. However, to be eligible, the proposed work must include activities or services within Linn County.
  • ​Lincoln, Benton, or Regional: Organizations may apply for funding to support work that occurs specifically in Lincoln County, Benton County, or across the broader region. To qualify, the proposed work must include activities or services within either Lincoln or Benton County.​

Application Requirements 

Applicants must submit a complete proposal that includes:
  • Organizational Information: Name, mission, and relevant experience.
  • Project Description: A detailed description of the proposed initiative, including alignment with R-CHIP goals and strategies.
  • Target Population: Description of the population served, including efforts to address disparities.
  • Implementation Plan: Workplans, measurable objectives, and expected outcomes.
  • Collaboration: Description of partnerships and community engagement efforts.
  • Budget: A detailed budget with justification for requested funds
  • Evaluation Plan: Metrics to measure success and impact. ​

Grant Agreements and Reporting

Grantees who are awarded funding are subjected to sign an annual grant agreement, as well as, submit an interim and annual report. More details will be provided in the grant agreement and reporting once they become available.

FAQ

What is the purpose of the Modernization Grant?
The grant is designed to support community-based organizations in Linn, Benton, and Lincoln Counties in advancing health equity by implementing programs aligned with the Regional Community Health Improvement Plan (R-CHIP). Funding supports either programmatic work or capacity building.
What types of grants are available and how much funding is offered?
  • Programmatic Grants – $25,000 (4 awards): For direct service delivery, outreach, or community education that address R-CHIP priority areas.
  • Capacity-Building Grants – $50,000 (2 awards): For enhancing organizational infrastructure, such as staff training, technology upgrades, or strategic planning.
Can funding be used for both new and existing programs?
Yes, as long as the project aligns with R-CHIP goals and includes measurable outcomes, funding can support either new initiatives or enhance existing programs.
Can my organization apply for both a Programmatic and a Capacity-Building Grant?
No. Organizations are strongly encouraged to apply for only one grant category to ensure the most strategic alignment with their current needs and to maximize equitable distribution.
Can you give examples of eligible activities by grant type?
Yes.
  • Programmatic Example: A mobile behavioral health outreach initiative serving rural communities.
  • Capacity-Building Example: Upgrading an organization’s data management system to better track health outcomes across services.
How many grants will be awarded, and is funding limited?
Yes. Only six grants will be awarded:
  • Four at $25,000 (Programmatic)
  • Two at $50,000 (Capacity-Building)
    Due to the limited funding pool, not all qualified applications will be funded. Applicants should clearly demonstrate alignment with R-CHIP and propose high-impact, feasible projects.
